The 2016 Wellness Program features financial and health wellness opportunities on campus, including educational programs, walks/runs, and Blue Cross Blue Shield rewards to assist employees in leading a healthy lifestyles and financial wellness.
Benefit-eligible employees who complete five wellness activities from January to May 2016 and submit the rewards form are eligible for a $100 wellness reward. Activities include:
Receive preventive care services, such as:
- Annual physicals
- Breast exams/mammograms
- Prostate exams
- STI testing
- Smoking cessation
- Obesity screening and counseling
- Pap smears
- Domestic violence screening and counseling
- Screenings for diabetes and other lab procedures
Preventive care services are covered at 100% when employees are enrolled in the Loyola Advantage PPO Plan and are covered 100% by the Loyola Advantage PPO Plan when using a network physician. Please keep in mind that if any diagnoses are made during the wellness exam, the wellness exam is no longer covered at 100%.

The Well on Target® is a portal for Loyola Advantage BCBSIL PPO Plan members, offering personalized tools and resources to help plan your health and wellness path. Up to three Well on Target® activities, which include monthly challenges, health risk assessments, and online courses, can count toward the five total Loyola Wellness Program activities required for the $100 benefit. To login, visit the Well on Target® at www.wellontarget.com. Loyola Advantage BCBSIL PPO Plan members can access and enroll in monthly challenges during the first week of each month.
Gym Discounts—Sign up at a participating gym through the Well OnTarget program and you can receive discounted memberships and your fees may be waived. Please read the flyer for more details: Well on Target Fitness Program.
