New Wellness Program for 2016
The 2016 Wellness Program will feature financial and health wellness opportunities on campus. Planned events include educational programs, walks/runs, and Blue Cross Blue Shield rewards to assist you in leading a healthy lifestyle and financial wellness. Benefit-eligible employees who participate and complete five wellness activities, and submit the rewards form, are eligible for a $100 wellness reward. Employees are required to complete various wellness activities from January to May 2016. The dates and times of various activities will be added to our website on a monthly basis. This way employees will complete activities over the course of 5 months instead of in one sitting. Please see attached form and Wellness Activity Schedule for more details.
How the 2016 program works: Complete at least 5 Wellness Activities between January and May of 2016 in order to be eligible for the reward. Only employees who are enrolled in the Loyola Advantage Blue Cross Blue Shield (BCBS) PPO HealthPlan will have access to participate in the Well on Target® activities.
2016 Wellness Activities: Receive Preventive Care Services, attend Wellness Seminars, participate in Well on Target® Monthly Challenge, Well on Target® Health Risk Assessment and Well on Target® Online Courses.
General Preventive Care Services: Preventive care services are covered at 100% when you are enrolled in the Loyola Advantage PPO Plan. A list of all included preventive care services will be provided on the 2016 Wellness Program web page and are covered 100% by the Loyola Advantage PPO Plan, when using a network physician.
Wellness Seminars: 2016 Wellness Seminars are provided by Perspectives and Transamerica. Each seminar will count as a wellness activity. Please note that dates and times below are subject to change.

- Just log in to Blue Access for Members at bcbsil.com/members. If this is your first time logging in, you will need to register your account.
- Click the Well on Target link to access the portal
- Once you are logged in to Blue Access for Members, simply click the link on the right side of the page and it will take you to the Well on Target portal, where you will find personalized tools and resources to help you plan your health and wellness path.
