Appointments & Services

All nutrition appointments for the Wellness Center must be made by calling or online. The Wellness Center does not accept walk-in appointments. To schedule an appointment please call Dial-A-Nurse at 773.508.8883 or schedule online.

*Please arrive 15 minutes early to initial appointments in order to complete screening surveys upon check-in.*

UPDATE: As the Wellness Center continues to follow CDC Guidelines for the COVID19 pandemic, nutrition appointments can be conducted in person or via videoconferencing.

The Registered Dietitian will provide you with a unique zoom link prior to your scheduled appointment time if videoconferencing is preferred.
