File an Incident Referral

All members of the Loyola community, including non-Loyola partners and neighbors, have the right to report alleged student misconduct to the University. Additionally, all members of the Loyola community have a right to request conflict resolution resources. All incident referrals will be reviewed by university staff and, where potential violations of the Community Standards are found or where there is a request for conflict resolution services, staff will follow up.  

Although a person submitting an incident referral is not required to provide any personal information, we HIGHLY recommend that you provide, at a minimum, an email address where you could be contacted. This is especially important if there are follow up questions or responses that may be important or necessary. If a reporter fails to provide any identifying or contact information, it may significantly limit the University's ability to look into or fully respond to a situation. The privacy of the person making a report, along with the contents of the complaint, will be maintained to the extent that the university conduct process and conflict resolution processes allow.  

To best equip the University to respond to an incident or provide support related to a conflict, incident referrals should... 

  • Contain the name, phone number, and email address of the individual filing the incident referral; 
  • Contain a detailed, factual account of the behavior being reported (including date, time, location); and
  • Include any supporting documentation (attached with the incident referral) that is relevant to, or substantiates the information provided in the report such as photos, videos, screenshots, or emails.
