Clearance Instructions

SSOM clearance instructions for excused absence from class/clinicals: 

  1. Call Dial-A-Nurse at 773.508.8883 upon symptom onset (please do not wait as we cannot provide retroactive clearance). We answer calls and return voicemails as they come. If we don't answer, you must leave a detailed voice message, and a nurse will call you back as soon as possible.
  1. Take a home covid test and write your name, date of birth, and the date and time of the test on the home covid test cassette.
  1. Email a photo of this to wellnesscenter@luc.edu
    1. If the test is NEGATIVE, you will be required to take a second home covid test 48 hours later. Take a photo of the cassette with the same information and email a photo of this to wellnesscenter@luc.eduSSOM requires students to have two negative home covid tests 48 hours apart in order to return to campus for any one covid symptom.
    1. If the test is POSITIVE,  take a photo of the cassette with the same information and email a photo of this to wellnesscenter@luc.edu
      1. You will need to isolate for five days. Once you test positive for covid, repeat testing is not accepted to reduce your isolation days. You can return on day six after clearance is obtained. You must continue to mask indoors around others for 10 full days.
  1. After your second negative covid test, call Dial-A-Nurse to make an appointment for clearance to return to campus
  1. If you test positive for influenza, you will be required to isolate for seven days per the LUMC policy. You can return on day seven after clearance is obtained. You must continue to mask indoors around others for 10 full days.
  1. Students are required to have an excuse note for missing any classes/clinicals. All clearances must be done through Loyola Wellness*
  1. If you have any questions, please call Dial-A-Nurse at 773.508.8883

* If you need a clearance to return to class/clinicals when the Wellness Center is closed, you can go to another provider to request a clearance note (primary care, urgent care, minute clinic, etc). That appointment would be through insurance and may require a copay or an office visit fee. This provider cannot be a relative.
