Emotional Support Animal Request

An individual may qualify for an emotional support animal if they are a person with a disability, the animal is necessary in order to provide the personal an equal opportunity to use and enjoy on-campus housing, and there is an identifiable relationship between the disability and the assistance the animal provides. 

In order to be approved for an emotional support animal, students must be registered with SSWD. Emotional support animal registration must be completed at least 30 days prior to the beginning of the academic semester for which the request is being made. 

To register with our office, students must:

1. Complete an Intake Form

  • Please download the form to save a copy.
  • Please submit via email to sswd@luc.edu. You may also fax or mail the form, but email is the preferred method. 

 2. Submit Documentation

  • Documentation guidelines can be found here
  • Any documentation must highlight the necessity for an emotional support animal and the ways that it alleviates the symptoms of the student’s disability. SSWD reserves the rights to request further documentation, if necessary. 
  • Please submit via email to sswd@luc.edu. You may also fax or mail the form, but email is the preferred method. 

 3. Meet with a Disability Specialist

  •  SSWD staff will contact you to set up an appointment once we have received all information from Steps 1 and 2.

 4. Additional Steps - After meeting with a Disability Specialist, if an emotional support animal is approved there are additional steps: 

  • Student will provide appropriate documentation, vaccination records, and a health summary from a veterinarian. 
  • SSWD will coordinate with Residence Life regarding any housing or roommate concerns. 
  • Any additional steps deemed necessary and in accordance with state and federal laws.

Emotional support animals are not permitted on campus until registration is complete. The student will receive final approval notification that they may bring the animal to campus from Residence Life.
