Open Enrollment is Now Open; Save on Premiums
Dear Faculty and Staff,
The 2018 Benefits Open Enrollment period is now open through Wednesday, November 22. This is your annual opportunity to review and make changes to your benefits. Detailed information summarizing benefit plans and changes are included in the 2018 Benefits Highlights Booklet.
During Open Enrollment, you can:
- Enroll in or change your medical, dental, or vision plans
- Enroll or disenroll dependents
- Enroll or re-enroll in the Health/Dependent Care Flexible Spending Account (FSA); re-enrollment is required annually.
Open Enrollment is a paperless process. Faculty and staff members are strongly encouraged to go through Open Enrollment to review benefits and certify their spouse and tobacco certifications via Employee Self-Service by November 22, 2017. To do so, users must be connected to Loyola’s network on campus or via Loyola Secured Access (LSA). For your reference, Employee Self-Service is compatible with all internet browsers, except Microsoft Edge.
Save on Your Health Insurance Premium
If you are enrolled or will be enrolling in the Loyola Advantage PPO 1 or PPO 2 plans, don’t forget to sign up for a biometric screening through Health Maintenance Institute (HMI) with the site code L773. You can save $50 each month on your 2018 health insurance premium by participating in the wellness program and biometric screening. New this year, employees and covered spouses or legally domiciled adults (LDA) are required to participate in the wellness screening and Health Power Assessment to receive the reduction. Both must be completed by November 22 to receive the reduced premium.
On-Campus Biometric Screenings and Free Flu Shots
There are still opportunities for those who are enrolled or will be enrolling in the Loyola Advantage PPO 1 or PPO 2 to receive biometric screenings on November 13 at the Damen Student Center or on November 17 at Lewis Towers. All faculty and staff can also walk in and receive flu shots from 7:30 a.m. to 12 p.m.
Faculty and staff members covered under Loyola University Chicago’s medical plans need to complete the Tobacco Use Certification form in Employee Self-Service by November 22.
Faculty and staff members who have a spouse/LDA covered under Loyola University Chicago’s medical plans need to complete the Spousal/LDA Premium Acknowledgement form in Employee Self-Service by November 22.
