MAKE WELLNESS A PRIORITY AND EARN $100

2013 Wellness Incentive Program

Our 2013 Wellness Incentive Program has begun. All Loyola University Chicago employees who participate in the Loyola Advantage Blue Cross Blue Shield (BCBS) PPO Health Plan are eligible for a $100 cash wellness reward. In order to be eligible for the cash reward, you have to complete FOUR of the SEVEN wellness initiatives listed below.

  1. Biometric Screening
  2. Health Risk Assessment
  3. Health Risk Assessment-Spouses
  4. Annual Physical
  5. ONE Age/Gender Appropriate Preventative Test
  6. Online Smoking Cessation Program
  7. Annual Dental Exam

Cash rewards will be paid out quarterly beginning April 2013To collect your cash reward, you should complete the wellness form and send the form to Human Resources. For those who have completed the on campus Biometric Screenings during our Health Days in November 2012 no provider signature is needed. No other services provided in 2012 outside of the Biometric Screenings in November 2012 will count towards your 2013 wellness incentives. All other services MUST BE rendered in the 2013 calendar year.  Employees have until December 31, 2013 to submit their 2013 Wellness Incentive Reimbursement form. Simply complete, sign and return the form. BCBSIL will provide proof for all services rendered with the exception of your Annual Dental Exam. Health Risk Assessments for both Employees and their Spouses and the Online Smoking Cessation Program can be accessed via BCBSIL Well onTarget Wellness Website. 

Rewards will be processed and paid through the payroll system and will appear as part of your paycheck (Note: cash rewards are taxable).

How To Access Well onTargetSM Online Tools

There are two ways to get to Well onTarget and begin using the personalized wellness resources.

 • Log in to your Blue Access for MembersSM account at www.bcbsil.com and click Well onTarget in Quick Links.

 -or-

 • Got to wellontarget.com and use your Blue Access for Members username and password to log in.

First time users of Well onTarget and Blue Access for Members will need to create a username and password to access the Liveon member wellness portal. Select Register Now on the log in page and follow the instructions. Make sure you have your BCBSIL member ID card available as you’ll need to enter some numbers from it.
