• Summer Meal Plan Request Form

This form is designed for students attending classes over the summer semester. Summer Meal Plan Request Forms will be accepted from May 1st, 2013 through July 5th, 2013.

• First-Year Resident Meal Plan Change Request Form

This form is designed for first-year resident students who are interested in changing their meal plan to best fit their needs.

First-Year Resident Meal Plan Change Request Forms for the Spring 2013 term will be accepted from October 22, 2012 through January 18th, 2013.

• Upper-Class (Apartment Style) Resident Meal Plan Change Request Form

This form is designed for upper-class resident students who are interested in changing their meal plan to best fit their needs.

Upper-Class Resident Meal Plan Change Request Forms for the Spring 2013 term will be accepted from October 22, 2012 through January 18th, 2013.

• Non-Resident Meal Plan Request Form

This form is designed for non-resident students who are interested in having a meal plan for use at on-campus dining facilities.  Non-Resident students must submit a request each term they wish to have a meal plan.

Non-Resident Meal Plan Request Forms for the Spring 2013 term will be accepted from October 22, 2012 through April 12,2013.

• Charge Authorization Form

This form is intended for students to add Rambler Bucks to their campus card for use in Loyola's Follett Campus Bookstores and other locations Rambler Bucks are accepted. The form authorizes the Office of the Bursar to charge the amount requested onto one's student account. 

Charge Authorization Forms for the Spring 2013 term will be accepted from December 1st, 2012 through January 25, 2013.

Charge Authorization funds will be placed on ID cards on January 3, 2013.

Charge Authorizations are not available for summer terms.
